Document Updates: VA Form 26-1820 Report and Certification of Loan Disbursement (Cx52), and VA Form 26-8497 VA Request for Verification of Employment (Cx17513)

After a recent audit of our VA forms, the following changes were made to Cx52 and Cx17513 to more closely align with the model forms provided by the VA:

Edits to Cx52 – VA Form 26-1820 Report and Certification of Loan Disbursement:

  • Page 1/Top Right Header: Addition of “Expiration Date: 09/30/2020” underneath ‘Respondent Burden: 15 minutes” text.
  • Page 1/Box for “Instructions to Lenders:” Font size reduced to conform with Federal model, including minor grammatical edits.  
  • Page 1/Bottom Left Footer: Line that reads “SEP 2012” to be replaced with “SEP 2017.” This is a universal change throughout the document; wherever “SEP 2012” appears, it will be replaced with “SEP 2017.”
  • Page 1/Bottom Middle Footer: Line that reads “SUPERCEDES VA FORM 26-1820, DEC 2007” to be replaced with “SUPERCEDES VA FORM 26-1820, SEP 2012.”
  • Page 2/Paragraph “O”: Line that reads “Any deviations of changes…” to be replaced with “Any deviations or changes…”
  • Page 3/Paragraph “f:” Fourth sentence down that reads in part “… is illegal and void and civil action for preventative relief….” to be replaced with “… is illegal and void and civil action for preventive relief…”
  • Page 3/Bottom of page Box: Language in italics that reads in part “Federal Statutes provide severe penalties for any fraud, international misrepresentation,…” to be replaced with “Federal Statutes provide severe penalties for any fraud, intentional misrepresentation,…”
  • General Appearance: Edits were made throughout document to more visually align with the model form provided by the VA.

Edits to Cx17513 – VA Form 26-8497 Request for Verification of Employment:

  • Page 1/Top Right Header: Addition of “Expiration Date: 09/30/2020” underneath ‘Respondent Burden: 10 minutes” text.
  • Page 1/Box with Paragraphs for “Privacy Act Notice” and “Respondent Burden:” Minor grammatical errors as well as the replacement of link whitehouse.gov/omb/library/OMBINV.VA.EPA.html#VA with www.reginfo.gov/public//do/PRAMain.
  • Page 1/Box for “Part II – Verification of Present Employment:” In Box marked “OTHER,” the word “(specify)” will be added in italics and parentheses to follow “OTHER.”
  • Page 2/Bottom Left Footer: Line that reads “DEC 2007” to be replaced with “SEP 2017.” This is a universal change throughout the document; wherever “DEC 2007” appears, it will be replaced with “SEP 2017.”
  • Page 1/Bottom Middle Footer: Line that reads “SUPERCEDES VA FORM 26-8497, OCT 2004” to be replaced with “SUPERCEDES VA FORM 26-8497, DEC 2007.”
