Following a standard review, we have updated SSA-89, Authorization for the Social Security Administration To Release Social Security Number (SSN) Verification (our Cx5579), to reflect the most recent version of the form (revision date of 04-17). The new SSA-89 version contains a more concisely worded Privacy Act Statement. Custom SSA-89 copies are also being updated to the new version.
